Introduction

L’obésité et la surcharge pondérale sont des facteurs modifiables associés à l’arthrose et constituent des leviers importants en matière de prévention, en l’absence de traitement curatif de cette pathologie [1]. L’objectif principal de notre travail était de décrire la prise en charge nutritionnelle des patients obèses souffrant d’arthrose.

Méthodes

Il s’agissait d’une étude rétrospective à visée descriptive portant sur 92 dossiers de patients obèses souffrant d’arthrose, suivis dans le service de Nutrition de l’INSP d’Adjamé sur une période de sept (07) mois.

Résultats

Au total, 92 patients ont été inclus, dont 82,60 % de sexe féminin et 17,40 % de sexe masculin. L’âge moyen était de 50,89 ± 12,38 ans.
Les localisations de l’arthrose concernaient principalement les genoux (80 %) et le rachis lombaire (20 %).

L’indice de masse corporelle (IMC) se répartissait comme suit :

  • 30 à 35 kg/m² : 39,13 %

  • 35 à 40 kg/m² : 28,26 %

  • 40 kg/m² : 32,61 %

L’évaluation nutritionnelle retrouvait un régime hypercalorique chez 83,49 % des patients, hyperprotidique chez 89,45 % et hyperglucidique chez 53,26 %.
Un régime hypocalorique a été proposé à l’ensemble des patients. Des régimes spécifiques ont également été prescrits : hypocholestérolémiant (39,13 %), hyposodé (35,86 %) et hypouricémiant (7,60 %).

L’adhésion initiale aux propositions diététiques était totale. Toutefois, un refus d’inclusion de certains aliments a été noté, notamment le soja (9,78 %), les lentilles (10,89 %) et les petits pois (5,43 %).
La motivation au suivi en consultation diminuait chez 79,34 % des patients après un mois. Parmi les douze patients revus en consultation, une réduction de l’IMC a été observée, avec une diminution moyenne de 8,96 kg/m².

Conclusion

La prise en charge nutritionnelle des patients obèses souffrant d’arthrose nécessite une évaluation nutritionnelle rigoureuse, intégrant le niveau socio-économique et la culture alimentaire, afin d’améliorer l’observance et l’efficacité du suivi.

Abstract (English)

Introduction

Obesity and overweight are modifiable factors associated with osteoarthritis. In the absence of curative treatment in osteoarthritis, they are interesting factors for prevention [1]. The main objective of our work was to describe the nutritional management of obese patients with osteoarthritis.

Methods

Retrospective study with a descriptive aim covering 92 files of obese patients suffering from arthritis followed in the Nutrition Department of the National Institute of Public Health (INSP) of Adjamé over a period of seven (07) months.

Results

Ninety-two patients were included in the study, 82.60% of whom were female and 17.40% male. The mean age was 50.89 ± 12.38 years. Osteoarthritis affected the knees (80%) and the lumbar spine (20%).

The distribution of patients according to body mass index (BMI) noted a BMI between 30 and 35 kg/m² (39.13%), between 35 and 40 kg/m² (28.26%) and greater than 40 kg/m² (32.61%).

Eighty-three patients (90.22%) were not following a specific diet. Nutritional evaluation was noted a high-calorie, high-protein, and high-carbohydrate diet in 83.49%, 89.45%, and 53.26%, respectively.

Adherence to the diet proposal was total in all patients. A refusal to include soy, lentils, and peas was noted in 9.78%, 10.89%, and 5.43%, respectively.

The low-calorie diet was proposed to all patients, the hypocholesteremic diet to 39.13%, the low-sodium diet (35.86%) and the hypouricemic diet (7.60%).

The motivation for follow-up in consultation was down in 79.34% after 1 month. The evaluation in the twelve patients (12.56%) reviewed in consultation the first month noted a reduction in BMI with an average of 8.96 kg/m².

Conclusion

Nutritional management of obese patients with osteoarthritis requires a rigorous nutritional assessment taking into account the socioeconomic level and food culture in order to obtain better compliance.
